Cannot create New Action for a work order

I am unable to create a new action for the workorder, when I am trying to enter what th resolution was.

How dp I create a new Action Item?

When closing a work order, where do I put the information of what was done to resolve the problem?

Action is tied to RCM module. Steps to create action:
1. Go to RCM module
2. Create a new failure
3. Open the failure form and click on action tab to add a new action

I did as prescribed, but when I open a work-order and try to click on the action selection icon, there are no selections to select from even though failure codes are present...

Couple things to check:
1. Open RCM's failure list view and double click on a failure code to open failure form. Verify that action tab has an action code defined.

2. Close work order edit form, close work order form (by work order form I mean the form with multiple tabs).

3. Open work order list form, double click to open the work order form.
Click edit button, now click on the lookup button to select action. You should see the action code displayed in step 1.

The action lookup form is missing a refresh button at this time and will be added at the upcoming release which is around the end of August.
